Navigating your health care alone can be confusing at the best of times; when faced with a crisis it is often overwhelming.

HealthPilot is here to help.  We match you with a dedicated case manager (we call them Pilots) who understands the system and how to navigate it. 

Our Pilots are all licensed healthcare professionals who play the role of your favourite niece, doting grandson, or organized and loving child. Simply put, your Pilot is an expert health care navigator who acts as your advocate, with only your best interests in mind.

Medical Case Management

Our Pilots are all licensed healthcare professionals who are passionate about optimizing patient outcomes and are solely focused on delivering an excellent client experience.
Pilots are selected based on a variety of factors including qualifications, experience and personal values. Each of our Pilots are vetted with a background check, and are also supported with extensive training to ensure our clients and their families receive the best possible support.

At this time, HealthPilot offers services in the province of Ontario.

We are planning to expand in the future – receive updates and follow our journey on social media.

Currently the matching and intake process takes approximately 1-3 weeks, based on a variety of factors including Pilot fit and availability. We do our due dilligence to ensure each client is matched with a Pilot that best meets their unique needs. If you require a Pilot on an urgent basis (for example Bedside Advocacy), please indicate this on the Contact Us form.

We are committed to ensuring a good connection between our clients and Pilots.  In the event this is not the case, we will engage in the matching process again until a the right fit is found.  

Every path is unique to you, depending on your needs and goals. Services are designed on a case-by-case basis, including:
1. Intake Session. One-on-one comprehensive intake session with your Pilot, ensuring we understand and document your health history and health goals.
2. Action Planning. A personalised action plan that outlines the supports that will be provided, and the path you and your Pilot will take, together.
3. Advocacy and Navigation. Whether its helping book an appointment, advocate for you in an appointment or follow-up with a specialist about your place on a waitlist, your Pilot is working behind the scenes on your behalf.
4. Research and Translation. Your Pilot will review your diagnosis, manage your case as required and help you understand your options with patience and care.
5. Liaison. Think of your Pilot as the connective tissue between your various doctors and care providers, as well as family and friends that you want in the loop. Equally adept at stick handling medical professionals or challenging family dynamics, this can be an invaluable resource that saves you time, frustration and anxiety.

After the comprehensive intake and goal-setting sessions, you and your Pilot will set up an action plan. Most engagements include a virtual check-in with your Pilot every two weeks to discuss updates and share progress against your goals.

We will ask you to complete a confidential intake form to help us understand your health history, primary concerns and goals. Then your Pilot will contact you to set up a 1:1 video call, and together you will chart a path to optimal health.

Yes! We absolutely recommend that at the beginning that a family member or friend participate in the intake and goal setting process. Then, going forward, we will happily share updates with anyone you would like to be included on your journey.

HealthPilot itself is not part of the professional medical community, however our Pilots are all licensed healthcare professionals that work to give you advice, support and guidance. Any and all advice given is strictly that and should be discussed with your family doctor or specialist team.

You will have access to your Pilot via email and phone as you partake in bi-weekly meetings with them.

Your Pilot is a licensed healthcare professsional who is passionate about client advocacy and helping others navigate the healthcare system. By having a Pilot you can be sure to have a partner who cares about your wellbeing and who will spend time reviewing your health details in depth, in order to help you navigate the healthcare system.
